FDA's patient and consumer notice on regenerative medicine states that it has received reports of BLINDNESS, TUMOUR FORMATION, neurological events, bacterial infections including life-threatening blood infections, unwanted immune reactions, and cells migrating and forming unintended tissue, following the use of unapproved regenerative medicine products - a list that explicitly includes stromal vascular fraction, amniotic fluid, Wharton's jelly, ortho-biologics and exosomes. A meta-analysis of 6 RCTs (860 patients, 334 receiving BMAC) found overall complication rates of 41.91% for BMAC versus 41.25% for comparator injectables (P=0.85), with knee effusion the commonest BMAC complication at 18.26%. Early and late complication rates did not differ significantly from HA, steroids, PRP, SVF, MSC or saline. The number needed to harm for BMAC relative to other injections was 152.

4. A systematic review of 32 studies comparing leukocyte-poor and leukocyte-rich PRP in knee OA found both formulations improved pain and function above the MCID with no significant difference between them at 3, 6 or 12 months - but leukocyte-RICH PRP carried significantly higher odds of post-injection pain (OR 1.64; 95% CI 1.29-2.10) and swelling (OR 1.56; 95% CI 1.22-1.99).

5. The AAOS patient-education FAQ on orthobiologics states that because orthobiologics are relatively new the evidence supporting their use is LIMITED, that rigorous testing of effectiveness in most orthopedic conditions is lacking, and that preliminary results are encouraging but hard to evaluate. It names tendinopathies such as tennis elbow, pain from early knee osteoarthritis, adjunct healing after rotator cuff repair, and avascular necrosis as the settings where biologic therapies have shown promise, and notes that stem cell treatments not derived from the patient's own body and further manipulated in a laboratory can only be offered inside an FDA-approved clinical trial.

6. The BMJ meta-analysis of viscosupplementation pooled 169 trials (21,163 participants) and found clear evidence of small-study effects and publication bias. In the prespecified main analysis of 24 LARGE placebo-controlled trials (8,997 participants) hyaluronic acid reduced pain by SMD -0.08 (95% CI -0.15 to -0.02), equivalent to 2.0 mm on a 100 mm scale - far below the -0.37 minimal important difference - and trial sequential analysis showed there has been CONCLUSIVE evidence of clinical equivalence to placebo since 2009. Fifteen large trials showed a significantly higher risk of serious adverse events (RR 1.49).

7. The largest head-to-head trial of cell-based orthobiologics to date randomised 480 patients with KL II-IV knee OA across four arms: autologous bone marrow aspirate concentrate, autologous adipose stromal vascular fraction, allogeneic umbilical cord tissue-derived MSCs, and a corticosteroid injection control. At 12 months NONE of the three orthobiologic injections was superior to another or to the corticosteroid control on either co-primary endpoint (VAS pain, KOOS pain), and none of the four groups showed a significant change in MRI osteoarthritis score from baseline. No procedure-related serious adverse events were reported.

## What is PRP made from?

PRP is made from your blood. Staff spin it to gather more platelets, the small blood parts that help clotting, then place the concentrated blood into the sore joint with a needle.

## What can go wrong after PRP?

The treated joint may ache or swell for a short time afterward. Fever, spreading heat, leaking fluid or quickly growing swelling isn't something to watch at home.

## What does a marrow collection add?

Marrow is soft tissue inside bone. Staff use a needle to draw it from the pelvis, prepare it, and place it into the sore joint, so the pelvis may also ache.

## What trouble can gel cause?

Joint gel is thick fluid placed into the knee with a shot. It may bring little relief and can cause soreness or swelling, so ask whether another visit will be needed.

## How much walking can follow PRP?

Walking rules depend on the treated joint and how staff placed the PRP. Ask separately about walking at home, stairs, driving and sports because each activity works the joint differently.
